items that you should decide on before you meet with your Funeral Director, i.e., selection of casket, prayer cards, poll bearers, limousines, clothing, religious service, cemetery, etc.  All you need to do is read through each item, list everything you want to have done, make an appointment with your Funeral Home and give these personal choices to the Funeral Director. 

There is also a special "Notes" section which allows you to list any financial institutions, account #'s, mortgage information, computer passwords, special addresses, etc.
